  • Relationship of the Disturbed Balance between Matrix Metalloproteinases and their Tissue Inhibitors and the Invasion of Malignant Salivary Gland Tumours

    Objective To detect protein expression of MMPs and TIMPs in various salivary gland neoplasms and to investigate their roles in invasion and metastasis of the malignant salivary gland tumors.Methods Immunohistochemistry and Gelatin zymog- raphy analyses for MMP-2, MMP-9, MT1-MMP, TIMP-1 and TIMP-2 were performed in 26 malignant and 28 benign salivary gland tumors.Results The expression of MMP-2 and MMP-9 was significantly higher in carcinomas than in adenomas(P< 0·05). TheMMP-2/TIMP-1 andMMP-2/TIMP-2 was also significantly higher in carcinomas than in adenomas(P<0·05). There was a cooperated effect among MMP-2, MT1-MMP and TIMP-2. The expression of active MMP-2, proMMP-9 and active MMP-9 was significantly higher in malignant tumors than in benign tumors (P<0·05).Conclusion MMP-2 and MMP-9 may play im- portant roles in invasion ofmalignant salivary gland tumors. Adisturbed balance betweenMMP-2, MMP-9, TIMP-1 andTIMP-2 in malignant salivary gland tumors was detected. It was the absolute increase of MMP-2 and MMP-9 to induce the unbalance.

    Influence of the Expression of Heat Shock Protein 70 in Maxillofacial Squamous Cell Carcinoma by Themochemotherapy

    Objective To study the influence of themochemotheraphy on the expression of HSP70 in maxillofacial squamous cell carcinoma.Methods 12 patients were treated with themochemotheraphy twice a week,altogether 10 times.After 8 mg of Pingyangmycin infused, the patients were treated with microwave hyperthermia at 43℃for 40 min.The part of carcinoma tissue was removed with surgical operations at before treatment and aftre five times of treatment. The expression ofHSP70 in tumor cells was determined by SP immunohitochemcial method.Results The expression of HSP70 in tumor cells was enhanced obviously by thermochemotherapy.Conclusion Special high expression of HSP70 in the tumor cells was induced by thermochemotherapy. With the antigen presenting action and other action,HSP70 have special antitumor effect.

    Objective To study the distribution of the chromatic value of gingival porcelains.Methods The color change of Shofu VINTAGE and Vita VMK95 gingival porcelains on the black and white backings was determined with a spectrophotometer (PR-650).Results The results showed that the chromatic value of gingival porcelains was different on varying backings. The color space of gingival porcelain was L*: 41·85~50·64, a*: 12·46~19·46, b*: 1·33~4·62.Conclusion The chromatic value distribution cannot cover the color region of natural gingiva.

    Objective To study the expression and significance of p65 which is an important subtype of nuclear transcription factorκB (NF-κB) and its inhibitory protein IκBαin malignant transformation of hamster buccal mucosa.Methods The animal model of malignant transformation in hamster buccal mucosa induced by DMBA(0.5%) was established. Twelve paired specimens including normal buccal mucosa, epithelial hyperplasia, epithelial dysplasia and squamous cell carcinoma (SCC) were subjected to Western blot for the analysis of p65.The expression of IκBαwas observed in 22 normal buccal epithelia, 20 hyperplasia epithelia, 35 dysplasia epithelia and 23 SCC by immunohistochemical evaluation.Results In normal buccal epithelia and hyperplasia ep- ithelia, the expression of p65 was not obvious, and there was no significant difference between them(P>0.05).The expression of IκBαexisted at large, but mostly localizing in cell cytoplasmic staining in the basal cell layer and bottom of spinocelluar layer. With the occurrence of epithelia dysplasia,the expression of p65 gradually increased,comparing with normal buccal epithelia and hyperplasia epithelia(P<0.01). But the positive intensity of IκBαwas dramatically decreased(P<0.05). In SCC, p65 ex- pressed at a higher level comparingwith normal buccal mucosa and dysplasia(P<0.01), while the staining of IκBαwas feedback- ly higher comparing with dysplasia(P<0.01) and even with normal buccal mucosa(P<0.01).Conclusion NF-κB p65 is strongly activated in malignant transformation of hamster buccal mucosa. The abnormal expression of p65 and IκBα,especially the ascending expression of p65 as well as the descending expression of IκBαin dysplasia may be an early event during oral carcino- genesis, and can be used as biomarkers for supervising oral malignancy.

    Objective This research aimed to study and assess clinical application of the using of three Nickel-titanium (NiTi) rotary instruments, namely ProFile, ProTaper and Hero 642, in preparation of curved root canals.Methods  80 teeth with curved root canals were instrumented by ProFile, ProTaper or Hero 642 rotary instruments using crown-down technique in test groups and by stainless steel K files in control group. All teethwere obturated with lateral condensation method. The efficiency of preparation and obturation was analyzed with radiographs before, during and after operation.Results Three NiTi rotary instru- ments could keep the curvature and flow of curved canals very well, and the continuously tapered root canal shape was achieved. There were no transportation, apical blockage, and ledge in all test groups. The operative time was shorter for ProTaper than that of ProFile and Hero 642. Endodontic flare-up seldom occurred in test groups. Two rotary instruments fractured in canals.Conclu- sion With the NiTi rotary instruments, ProFile, ProTaper and Hero 642, curved root canals can be prepared effectively and quickly. Meanwhile, the problem of instrument separation should be prevented.
    Effects of Fluoride Drop or Fluoride Ion Infiltration in Toddler Period on Demineralization of First Permanent Molar of Children

    Objective To observe the effect of fluoride drop orfluoride ion infiltration on demineralization of the first permanent molar of children.Methods 45 children applied by fluoride drop over 3 years in Chengdu and 40 controls were selected. 45 children applied by fluoride ion infiltration more than 3 years in Xiamen and 40 controlswere selected in this study. Demineraliza- tion of the different sites of the first permanentmolarwas evaluated byKaVo DIAGNOdent. The effect of FD and FII in toddler pe- riod on demineralization of the first permanent molarwas evaluated.Results Early demineralization of the first permanent molar in children without fluoride drop were more severe than that of children with fluoride drop in toddle period(P<0·01). There was no significantly difference between demineralization of first permanent molar in children with fluoride ion infiltration and control group(P>0·05).Conclusion Fluoride drop in toddle period can help the remineralization of newly erupted first permanent molar of children. There is no effect of fluoride ion infiltration on demineralization of newly erupted first permanent molar of chil- dren.

    Objective Gingival crevicular fluid (GCF) analysis of various inflammatory mediators has been investigated as a means of providing objective criteria of peri-implanttissue health. In this report, the crevicularfluid levels of interleukin-8(IL-8), GCF volume and clinical parameters were studied.Methods GCFwas sampled from 35 healthy and 35 inflamed sites of implan- tation patients. IL-8 levelswere analysed using ABC-ELISA. Clinical parameterswere measured, and data analysiswas performed using the software package SPSS10.0.Results Significant difference was observed between healthy implant sites and peri-im- plantitis sites. GCF volumewas positively correlatedwith PD, PI, GI andMOB. The total amount of IL-8 was positively correlated with PD and GI.Conclusion This investigation suggested thatGCF volume and IL-8 cytokine may be of value as a diagnostic and prognostic marker for peri-implantitis.
    Effect of Early Treatment with the Twin-block Appliance

    bjective To evaluate the effectiveness of early treatment with the Twin-block appliance for the developing Class Ⅱdivision 1 malocclusion.Methods ATotal of 30 children with ClassⅡdivision 1 malocclusion, 18(8 male, 10 female) out of which received treatmentwith theTwin-block appliance, the other 12 cases(6 male, 6 female) without treatment served as con- trol group. Cephalometric data were collected at the start and the end of the study and statistical analysiswere applied.Results  Except the factor of growth, treatment with the Twin-block appliance resulted in reduction of ANB (1·55°), overjet(5·46 mm) and correction of molar relationship(4·07 mm). These changes were due to the change of Pg/OLp,Co/OLp and mandibular length. The change of A/OLpwas not significant. The skeletal effect contributing to the change on overjet and molar relationwere 58% and 78% respectively.Conclusion Early treatment with the Twin-block appliance was effective in reducing ANB angle, overjet and correction of molar relationship. The changes were mainly profit from the favorable skeletal change of mandible, espe- cially from its length, while the effect on maxillary was not significant.

    Objective To study the incisal jawmorphology of subjectswith normal occlusion and to discuss the relationship be- tween different vertical facial skeletal types and different incisal jaw cluster types.Methods After studied by radiographic cephalometry, the incisal jawmorphology of 169 subjects with normal occlusion in early permanent dentition were analyzed by K- mean cluster analysis.Results ①Morphology of maxillary ormandibular incisal jaws could be divided into fourtypes separately, including divisionⅠ、divisionⅡ、divisionⅢand divisionⅣ. In maxilla or in mandible, the incisal jawmorphology of divisionⅠ was characterized by wideness and shortness. In contrast, divisionⅡwas characterized by thinness and length. As for divisionⅢ and divisionⅣ, both of twowere intermediate types between divisionⅠand divisionⅡ.②Inmaxilla, itwas found that divisionⅡ was the majority(86·49%)in the high-angle subjects, divisionⅢwas the majority(47·67%)in the average-angle subjects, and divisionⅠandⅢwere the majority(82·61%)in the low-angle subjects. Inmandible, itwas found that divisionⅡwas the major- ity(51·35%)in the high-angle subjects, divisionⅣwas the majority(48·84%)in the average-angle subjects, and divisionⅢand Ⅳwere the majority(69·57%)in the low-angle subjects.Conclusion Owingtothe obvious characteristics ofmorphology in each cluster types and in each vertical facial skeletal types, different clinic considerationsmust be put forwardwhenwewanttomove the incisor.

    Study on Patient Satisfaction with Two Type of Twin-block

    Objective To compare patient satisfaction with two type ofTwin-block by clinic complain questionnaire, treatment effect satisfaction questionnaire and total satisfaction questionnaire, and to analyse their related factors with patient satisfaction. Methods 56 patients were randomly classified into fixed-Twin-block (TB1) and unfixed-Twin-block (TB2). The complaint and satisfaction of patients was evaluated by clinic complain questionnaire, treatment effect satisfaction questionnaire and total satisfac- tion questionnaire after10 days and 6 months of treatment.Results (1)Therewas significant difference between the complaint of two groups in the sixth month, but there was no difference in the tenth day. (2) There was significant difference between the total satisfactory degree of two groups. (3) There was significant difference between the tooth-jaw effective index and the mandible ef- fective index of the two groups.Conclusion The satisfactory degree of fixed-Twin-block is superior to that of unfixed-Twin- block. The adorn fashion, treatment stage and functional position has influence on patient satisfaction.
    Arthroscopic Diagnosis for Intraarticular Adhesion of Temporomandibular Joint

    Objective To demonstrate the incidence ratio of intraarticular adhesion (ID), arthroscopic findings of adhesions were evaluated by means of its shape, location and degree. To evaluate the correlation between intraarticular adhesion and the fol- lowing index, including patients′age, locking duration and interincisal opening preoperatively.Methods 280 consecutive arthro- scopically examined joints in 251 patients were evaluated. Two surgeons discussed and recorded the shape, location and degree of adhesion nine parts of the temporomandibular joint (TMJ) upper compartment on the chart. The adhesion ratio in different regions and the relation between different grades and ID stages of adhesions were calculated. The patients with ID stageⅢ,Ⅳ, Vwere divided into two groups, one with adhesion, and the otherwithout. Interincisal opening, locking duration and age were compared between two groups usingt-test.Results Of all 280 joints, 157 joints had adhesions, which accounted for 56·07%.The adhe- sion of grade 1 accounted for 44·59%, grade 2 for 32.48%, grade 3 for 7·64%, and grade 4 for 15·29%. The proportions of different grades in different stage were as follows. The joints with adhesion in StageⅡaccounted for 10·81%, StageⅢfor 48·76%, StageⅣfor 75·61%, and StageⅤfor 77·78%. There were 223 patients who suffered from IDⅢ~Ⅴ. All of 223 patients were divided intothe groupwith adhesion (136 patients) and the othergroupwithout adhesion(87 patients). The patients′ age, interincisal opening and locking duration between two groupswere statistically significant difference(P<0·001).Conclusion  Arthroscopic findings confirmed that the incidence ratio of adhesionwas high, mainly occurred in the older patientswith longer locking duration and less interincisal opening. With the higher stage of ID, the adhesion′grades rise.

    Sentinel Lymphoscintigraphy in Oral Carcinoma: A Clinical Report of 10 Cases

    Objective To determine the clinical value of preoperativein vivolocation of sentinel lymph node (SLN) by lym- phoscintigraphy (LS) in oral carcinoma.Methods 10 patients with oral carcinoma were included in this study (7 male to 3 fe- male).99m Tc-dextron was injected peritumorally 24 h before surgery. Lymphoscintigraphy imageswere obtained in anterior and lat- eral views. SLNs were detected and removed with the aid of dye during surgery. A standard radical neck dissection was then per- formed, and all lymph nodes were analyzed byHE staining.Results SLNs were successfully detected in 10 regions of 8 patients via lymphoscintigraphy. Consistent with the result of LS, 15 blue-stained SLNs were identified by means of dye method in 8 of 10 regions which the LS images indicated.Conclusion This study suggests that preoperativein vivolocation of SLN via lym- phoscintigraphy should be helpful for the application of SLNB in oral carcinoma.

    Objective To investigate the status of the temporomandibular joint(TMJ) in patients with complaints of joint pain for improving clinical therapy.Methods Twentythree joints in twenty patientswho complained ofTMJ painwere examined radio- graphically and arthroscopically.Results There were at least one and more to six pathological changes could be found arthro- scopically in the 23 temporomandibular joints, which were different in some respects with radiographic findings.Conclusion  TMJ related pain may be associated with pathological alterations in the TMJ, and synovitis may be one of the causes ofTMJ pain.

    Objective To observe the effect of using polytetraflouroethylene(e-PTFE) tube with self-Schwann cells implanted to repair facial nerve defect.Methods Enzymatic digest method was used to get pure Schwann cells in short time. The e-PTFE membrane tube was used to bridge the 1·0 cm defect of facial nerve and pure self-Schwann cells were injected into the tube. As control group, the e-PTFE tube without self- Schwann cells was used in the same way. Electric physiological and histological ex- aminations were taken in different times.Results The effect of nerve regeneration of the experimental groupwas betterthan con- trol group at any time. The nerve conduction velocity of the experimental groupwas 29·70 m/s in the 16thweek, while the control groups was 23·00 m/s respectively at the same time.Conclusion It is possible to obtain sufficient active Schwann cells by en- zymatic digest method. Using e-PTFE tube to bridge the defect of facial nerve with self-Schwann cells implanted can get effect of nerve regeneration.

    Objective To evaluate the effect of static magnetic field on expression of bone morphogenetic protein-2(BMP-2) in the periodontal membrane of experimental periodontitis rat.Methods Experimental periodontitis of rat was formed by ligaturing the neck of rat teeth and feeding sugar of high concentration. The magnetwhich the intensity of magnetic field was 0·12 tesla was put intotheir cheeks. The ratswere sacrificed at 2ndday, 4thday and 7thday, respectively. Immuno-histochemical techniqueswere used to evaluate the change of BMP-2 expression.Results BMP-2 was mainly found in the plasma of fibroblast, osteoblast, ce- mentocyte and odontoblast separately. There was not clearly difference between the periodontitis group and the normal group, but BMP-2 in the experimental group treated with static magnetic field was higher than the two groups.Conclusion Static magnetic field plays an important role in repairing and remodeling of periodontitis.

    Objective The purpose of this study was to invest the effect of epidermal growth factor receptor monoclonal anti- body MAb225 on radiosensitivity of tongue squamous cell carcinoma cell Tca8113.Methods Tca8113 cells were treated with different concentrations of MAb225. Radiation dose survival curve was generated from clonogenic survival assay. SERD0and sur- vival faction (SF2) was analysed by single hit multi-target (SHMT) radiobiological model using RADMEDIC software. Nude mice with Tca8113 tumor xenografts were treated withMAb225, radiation treatment or both of them. Tumor responses were assessed by tumor growth delay, andttestwas used for statistical analysis.Results MAb225 enhanced the radiosensitivity ofTca8113 cells. SERD0of Tca8113 cells treated with 0·5 mg/LMAb225 was 1·23. The survival fraction of cells treated withMAb225 was signifi- cantly decreased. Tumor radioresponse could be enhanced by MAb225 (P<0·05).Conclusion MAb225 could enhance ra- diosensitivity of tongue squamous cell carcinoma cell.
    Influence of Vascular Cell Co-cultured with Osteoblastin vitroon Ability of Osteoblast

    Objective The purpose of this study was to observe the influence on proliferation of rabbit osteoblast (ROB) of rabbit vascular endothelial cell(RVEC) co-culturedwith rabbit osteoblast (ROB).Methods Rabbit osteoblasts and rabbit vascu- lar endothelial cells in ratio of 1∶0,1∶1,2∶1,4∶1were selected to co-culture.The function of osteoblasts was assessed by alkaline phosphatase(ALP) activity and osteocalcin(OC) assay.Results There was good compatibilitywhen osteoblasts and vascular en- dothelial cells were co-cultured. The activity ofALP and OC in the group of ROB and RVEC co-cultured in ratio of 2∶1 was higher than that of the other three groups.Conclusion In direct co-culture systemin vitro, RVEC can improve activity of ROB.
    Development and Study of Three-dimensional CT Scanning System for Dental Cast Measurement and Analysis

    Objective To develop a computer assisted CTscanning system for 3D dental cast measurement and to study its re- liability.Methods 20 dental stone castswere scanned by helical CTand captured a series digital sliced radiographs of them.The program which was developed byVisual C 6.0 language and run underthe Windows surroundings rebuiltthe 3D graphics and mea- sured the coordinate of the dental cast. These dental castswere scanned and measured totestthe reliability of the system, contrast- ingwith manual measurement.Results There was no significant difference between 3D CT scanning and manual measurement (P>0·05).Conclusion The system is a valuable computer assisted systemfor clinical diagnosis and treatment ofmalocclusion.

    Objective The purpose of this studywas to explore the possible principles for remodeling of periodontal tissues un- der mechanical stretching by investigating the effects of intermittent mechanical stretching delivered by a new invented type of cell stress machine on the proliferation kinetics of the human periodontal fibroblast cells (hPDLFs)in vitro.Methods hPDLFs were culturedin vitro. The 4~7th generation of hPDLFswere undergone mechanical stretching. All cellswere grouped as strained and unstrained (controls). Strained groups cells were gathered after 2,4 and 6 hours with the stress 1 000,2 000 and 3 000μstrain. Flow cytometry (FCM) was used to examine the ratio of DNA(S%) and the index of proliferation(PI).Results Mechanical force would lead to the change of hPDLFs proliferation. Within the experiment, both 1 000μstrain and 2 000μstrain could in- creasethe proliferation index(PI)(P<0·05). While under 3 000μstrain, DNA synthesis was significantly decreased(P< 0·05).Conclusion Some range of mechanical stretching accelerates hPDLFs proliferation. This research discusses the remodel- ing process of periodontal tissues undergoing the mechanical stretching on cytological view, and itmakes a basement for further re- vealing the mechanical strain induced periodontal biological principles. Also it provides a useful direction for clinical orthodontic therapy.

    Objective Human soluble tumor necrosis factor receptor(sTNFR) can interfere with the biological functions of in- terleukin-1, which may be appropriate to the treatment of periodontitis. The eukaryote expression vector of the human sTNFR gene must be constructed prior to conducting transgene therapy of periodontitis.Methods Both sTNFR gene and plasmid pcDNA 3·1(+) DNAwere digested withKpnⅠandXhoⅠ. After purification, the two fragmentswere ligated byTakaRa DNALigation Kit (Ver 2·0). This recombinant DNAwas then transformed intoE.ColiCompetent Cells JM109 and positive cloneswere select- ed on the LB agarose plate containing ampicillin (80μg/ul).Results Six single cloneswere indentified by double digestionwith kpnⅠandxhoⅠand two fragments with the size of 5·4 kb and 1·0 kb were produced as expected.Conclusion The sTNFR gene was successfully inserted into the eukaryote expression vector plasmid pcDNA3·1(+) by recombination technologyin vitro

    Investigation on Oral Lesions in 64 Chinese HIV/AIDS Patients in Guangxi Province

    Objective To investigate the prevalence, age and gender distribution and clinical features of HIV/AIDS oral le- sions in patients in Guangxi province, and to provide the epidemiological information for prevention and treatment of these diseases in the certain population.Methods Atotal of 64 HIV/AIDS patients were included in this study. All patientsHIV serum-status was confirmed in Guangxi Center of Disease Control (GXCDC). Oral examination was carried out by standardized specialists. HIV/AIDS orofacial lesionswere recorded and diagnosed usingthe EC ClearingHouse Criteria on Oral Problems related toHIVIn- fection (1992).Results Among the total of 64 HIV/AIDS patients included in this study, there were 53 males and 11 females, withmean age of 36·1 years. Candidiasiswasthe most common lesionwiththe pseudomembranoustype predominating.High prevalences of xerostomia, 11 oral ulceration and 7 HIV related periodontitis were noted. 6 Herpetic stomatitis and 3 herpes zoster, 2 oral hairy leukoplakia and 1 Kaposi′s sarcoma and 1 lymphadentitis alsowere found.Conclusion This study shows a high prevalence of candidia- sis, salivary gland disease. Maybe oral ulceration prevalence is not increased, but lesion severity is increased with more severe heperi- form or major RAU. It suggested thatHIV/AIDS usually shows oral lesion and partly can appear in early phase.

    Influence of Different Porcelain Surface Treatment Method on the Bonding of Metal Brackets to Porcelain

    Objective To investigate the influence of different porcelain surface treatment methods on the shear bond strength of metal brackets bonded to porcelain.Methods 80 porcelain facetswere divided randomly intotwo groups accordingto different adhesive material thatwas used to bond metal brackets. Adhesive materialwere Jing-Jin enamel adhesive and light-cured composite resin. Each group was further divided into 4 subgroups according to different surface treatment methods,which were acid etching with 37% phosphoric acid(H3PO4),acid etching with 9·6% hydrofluoric acid(HF),deglazing by grinding and silanating the porcelain surface.All specimenswere stored in 37℃waterfor24 hours and then the shear bond strength and the porcelain fracture after debonding was determined. The porcelain surfaces afterHF etching, H3PO4etching and deglazing by grindingwere examined by scanning electron microscopy respectively.Results The shear bond strengths in the HF etching groups, the deglazing groups and the silanating groups were much greater than that in the phosphoric etching groups(P<0·01). Adequate orthodontic bonding strength was achieved both when bonded with light-cured composite resin after deglazing by grinding and when bonded with either of these adhesives afterHF etching or surface silanating. There were no differences in the rates of porcelain fractures among groups (P>0·05).Conclusion HF etching, deglazing by grinding and silanating can all increase the shear bond strength between metal bracket and porcelain. Surface silanating of porcelain is a better surface treatmentwhen metal brackets bonded to porcelain.
