不同门诊护理宣教方法对门诊颞下颌关节紊乱病患者依从性及治疗满意度的影响

Effects of different education methods on compliance and satisfaction of the patients with temporomandibular disorders

摘要:

目的 比较常规护理宣教配合视频宣教或宣传单宣教与单纯常规护理宣教在改善患者治疗满意度及依从性中的效果。方法 将133例诊断为颞下颌关节紊乱病并接受关节腔内注射透明质酸钠的患者,随机分为3组:Ⅰ 组患者接受视频宣教及常规护理宣教,Ⅱ组患者接受宣传单宣教及常规护理宣教,Ⅲ组患者仅接受单纯常规护理宣教。治疗前记录患者的人口学特征(性别和年龄)及诊断,接受相应护理宣教后随访1月,记录患者对治疗的满意程度、对宣教的注意事项执行情况及治疗的依从性。结果  各组患者的性别、年龄及诊断均无统计学差异(P>0.05)。 133例患者中17例失访,Ⅰ组失访率为4.5%,Ⅱ组失访率为11.1%,Ⅲ组失访率为22.7%。3组患者的失访率有统计学差异(P=0.035),按时随访的情况也存在统计学差异,Ⅲ组患者按时随访率明显低于Ⅰ组和Ⅱ组(P=0.04)。Ⅰ、Ⅱ、 Ⅲ组患者的治疗满意度分别为90.5%、92.5%、76.5%,Ⅰ组、Ⅱ组的治疗满意度高于Ⅲ组(P=0.05)。3组患者对宣教措施的执行程度有统计学差异,Ⅰ、Ⅱ组对宣教措施的执行程度高于Ⅲ组(P=0.007)。结论 在传统的门诊护理宣教中加入视频宣教或宣传单宣教,能够明显提高患者的依从性及对治疗的满意度,同时引导患者建立良好的保护颞下颌关节的生活方式。

Abstract:

 Objective To compare the effects of video or leaflet in conjunction with traditional patient education (TPE) with traditional patient education alone for the compliance and satisfaction of the patients with temporomandibular disorder(s TMD). Methods  One hundred and thirty-three patients with TMD who needed intra-articular injection of hyaluronate were included and randomly divided into three groups: Group Ⅰ accepted video education plus TPE, Group Ⅱ accepted leaflet education plus TPE, Group Ⅲ accepted TPE alone. All the participants were recorded by demographic characteristics(sex, age) before treatment, and their compliance, satisfaction and self-exercise were also recorded after one month followed-up. Results  The baseline parameters(sex, age and diagnosis) of the groups were simila(r P>0.05). Seventeen participants lost in follow-up, including 4.5% in Group Ⅰ, 11.1% in Group Ⅱ and 22.7% in Group Ⅲ. There were significant differences in lost rates among the three groups(P=0.035). The rates of participants who exactly followed the appointed follow -up were higher in Group Ⅰ and Ⅱ than in group Ⅲ(P =0.04). And the satisfaction rates were 90.5% in Group Ⅰ, 92.5% in Group Ⅱ, 76.5% in Group Ⅲ. The satisfaction rates of Group Ⅰ and Group Ⅱ were significantly higher than that of Group Ⅲ(P=0.05). The patients in Group Ⅰ and Group Ⅱ were more compliant with the self-exercise than that in Group Ⅲ(P=0.007). Conclusion  Adding video or leaflet education to the TPE could increase the patients’compliance, satisfaction and execution of the self-exercise.
